What are the tank dimensions? I have had several fish do this to my medium/large clowns, including my kubs. it primarily has been younger yoyos and kubotai. the young clowns will do a similar, but less pestering version of this behavior that the older clowns tolerate. the young clowns seem to understand their place in the heirar...

the white fuzz makes me thing it was a bacterial or fungal infection that combined with the ich. The loss of color and fin rot on that fish also make it seem like more was wrong that just ich, even a really bad case of ich. the fuzzy food makes me feel like their could also be over feeding or water ...

Biggest issue with LED lighting is that it doesn't produce nearly as much UV as other forms of artificial light. Do you intend to have a lot of live plants? I also think my fish and inverts benefit from a little UV in their lives.
