Please Help!! My loach has lumps again!!!

My weather loach got ill a year or so ago and had some very nasty lumps develop on his body which eventually split the skin and a round, opaque ball shaped 'thing' came out. He was really sick and I looked for advice on here. He did get better and we had our 'eely' back. Here is a link to my previous thread, http://forums.loaches.com/viewtopic.php?f=1&t=22560
Today we noticed that he seems to have the same problem again but this time the lumps are bigger and there are more of them. He seems quite bad and keeps floating, finding it tricky to stay on the floor. He seems to have a small hole in his side too.
Can anyone recognise these symptoms and help me make him better again?

Sorry, I do not know what is going on with this fish.

Other causes of swelling:
Eggs: (female fish, localized in the abdomen, not in other places)
Dropsy: More of an overall swelling, more evenly distributed, not lumpy.
Tumor or other growth: This is one that could show up anywhere on the fish, but usually does not come up so fast and in so many places.
Parasite: I have no idea which one, but this is where I would start.
38 tanks, 2 ponds over 4000 liters of water to keep clean and fresh.
