Sorry for not getting back with an update.
It turned out that I was finally successful in killing off the ich with a combination of running the tank at 87F for two whole months, and installing a 36W UV sterilizer fed by a small fountain pump.
The tank has been running at 78F for nearly 2 years and all fish are apparently happy and healthy. I leave the UV going full-time as an ich preventative and because it prevents algae blooms.
Stress kills - and the fish don't like it either.
