Thank you so much for the informative response.

I was using a liquid nitrate test kit, and I didn't shake the bottle or the vial. The instruction manual had gotten wet at some point, so all of the pages were stuck together and ruined. I was going by the limited information on the bottle ...

The loaches are dojo loaches (weather loaches?). The sick one looked just like this:
