My vote is for pool filter sand hands down. You can't beat the price, the fish love it and best of all you don't have to rinse it before putting it in your tank. BTW...switch out your substrate gradually so you don't decimate your bio filter and cause an ammonia spike.

I'm inclined to think it's some sort of herniation. There's no redness to indicate infection and the blood vessels present look like normal anatomy. I've never see anything quite like this. Your kuhli is definitely a female and I wouldn't rule out something related to egg production. I bought 25 Kuhlis 2 weeks ago and only 4 are left. A month before that I bought 18 and only 8 survived. Increasingly over the past decade the survival rate of Kuhlis has dropped dramatically.
