help!, ill goldfish!!

one of my brother's goldfish is ill and we're a bit worried about it (understatement!)
it has something that looks like a lump (a BIG lump) on its side just behind its head and seems to have a little difficulty swimming (although it soon shifts when it thinks it's getting food!)
i'll try to get a camera to get some pictures up but i'm starting to worry about it since he only noticed it today and i don't know how long it has been this way although i don't expect it to have been in difficulties too long
i tested the water and it was fine (better than mine! >.<) so short of thumping into something REALLY hard and swelling up i really don't know what could be wrong!
any advice?
