Dojo with small lump on his back (PIC)

Hi,
I have two Dojo loaches about 5+” in size, both active, feed well and appear healthy
However one has developed a small lump on his upper back (see photo), he still appears well
Tank is mature, Ammonia 0, Nitrite 0, Nitrate 10ppm, PH 7.5, temp 23c
Any ideas ?
Update - White spots now appearing, these have now disappeared, however Gill movement is very fast and the loach is constantly trying to burrow into the substrate to relive/ scratch its gills
